Individual Monitored at an NRC Licensed Facility
To request a dose history report (NRC Form 4 or NRC Form 5) for an individual monitored at an NRC licensed facility (10 CFR 20.2206), or for an NRC employee, or for monitoring records for a DOE facility, you may now use the Automated Dose History Request Form.

Release Waiver for Privacy Information
Requests must be accompanied by a release waiver (organizational, individual) signed by the monitored individual. Records can be retrieved by name, but it is recommended that the request include the individual's social security number in order to ensure the accuracy of the retrieval. Requests that include the individual's social security number need to be protected in accordance with the Privacy Act of 1974 and should be marked as 'Privacy Act'.
