Flowcharts to help you fill out Form 5 - Including NR, ND, NC

The following images are simple, straightforward flowcharts to help you fill out Form 5 for reporting to the NRC. They are not a substitute for reading the entire contents of Regulatory Guide 8.7, Rev. 1, but they may help you to get a feel for how to deal with the dose codes NR, ND, and NC. Use your browser to print them and keep them handy for reference.

The flowcharts use the term "Gosub ..." to refer to subordinate flowcharts (remember the old days of real programming?). In the future we hope to add the ability to click on a flowchart item to hyperlink to the next level.
